Jr. Camp Registration Information

Registration opens on 5/1 at 8:00pm Central Time.

Please complete one registration form for EACH camper

Before proceeding, please note and agree to the following:

1. Camp fees include lodging, meals, transportation via bus to/from camp, and a camp T-shirt. Some optional activities and snacks are available for an additional fee (payable in cash upon arrival to the campsite; see FAQ on our website for more details):

Junior Camp Fees 2026

Register before June 1: $410*
Register June 1 to July 15: $430*
*Includes a $40 nonrefundable processing fee*

2. Full payment is due at time of registration and will be collected at the end of this form. Payment must be made by credit or debit card (paper checks are no longer accepted). Submitting this form without completing payment does not reserve a spot.

3. Refund Policy: Cancellations received by email on or before July 15 will receive a full refund minus the $40 processing fee.

No refunds will be issued after July 15 as we are billed by the campsite for each camper regardless of their attendance. Campers are not required to stay the full duration of the week (with Director approval), however no partial refunds will be issued for the same reason. Please contact us with any questions.

4. Per IRS guidelines, overnight camp fees are NOT considered tuition or childcare and cannot be submitted for FSA reimbursement or tax purposes.

5. If you require financial assistance, please complete the registration form, select “Financial Aid Needed” at checkout, and contact the directors for further information. Selecting “Financial Aid Needed” does not guarantee approval.

5. Additional FAQ and director contact info can be found on our website at www.ccucjuniorcamp.com/faq

JOIN THE WAITLIST:
During the first week of registration, we limit available spots so we can properly size our staff and cabin assignments. Joining the waitlist ensures your child is first in line as additional spots open, based on the timestamp of your form submission.

If your child's age group is full, please select the "WAITLIST" age group and complete the registration form. Payment is NOT REQUIRED to join the waitlist, simply submit the form and select "WAITLIST - $0" at checkout.

We will contact you if a spot becomes available and provide instructions for completing payment.
For questions, please email ccucjuniorcamp@ccuc.net.

Transportation Info:

Transportation to and from camp is provided by bus from CCUC (Chinatown) or CCFC (Wilmette). Please select how your child will arrive at camp and depart from camp.

If you plan to drive your child to or from the campsite, please contact the camp directors in advance. Other adults driving campers must be listed on the Authorized Pick‑Up List.

Health Info:

In accordance with Wisconsin ATCP 78.27(1)(d), all youth camps must collect a health history for every camper. This information is confidential and shared only with staff who need it to care for your child. Please complete the following section so our staff can provide safe and appropriate care.

A First Aid counselor will be on site and may assist your camper if needed. We provide basic over‑the‑counter medications such as Benadryl, Tylenol, Ibuprofen, and anti‑itch creams with parental consent. If your child requires specific medication, please be sure to pack it with them.

All prescription medications must be declared and provided in their original containers.
Prescription medications will be collected from campers upon arrival to the campsite. You do not need to list or declare over‑the‑counter medications that your child may self‑administer, such as Claritin, Allegra, or Tylenol.

Parent/Guardian Consent & Authorization

1. Emergency Medical Care
I authorize camp staff to obtain emergency medical treatment for my child if necessary, including evaluation and care by onsite medical personnel or outside medical providers, and to contact emergency services or transport my child to a hospital if needed. If applicable, I give permission for my child to self‑administer emergency medications such as epinephrine, inhalers, or insulin.

2. First Aid and Over‑the‑Counter Medication
I authorize camp health staff to provide routine first aid and to administer approved over‑the‑counter medications such as Benadryl, Tylenol, Ibuprofen, and anti‑itch creams as needed, or equivalent generics, unless I have opted out on this form. I also give permission for camp staff to provide routine healthcare treatment such as basic wound care.

3. Prescription and Personal Medications
I understand that all prescription medications must be declared and provided in their original containers. I authorize camp staff to assist with or administer these medications according to the instructions I provided.

4. Transportation & Release Authorization
I authorize my child to be transported to and from camp by bus as arranged by the camp, or by alternate means based on the selections I made on this form. I authorize the camp to release my child only to the parent/guardian(s) listed above or to the individuals I have designated in this form. I understand that any adult dropping off or picking up a camper may be asked to present a government‑issued photo ID, and that the camp may refuse release if identity cannot be verified.

5. Participation in Camp Activities
I give permission for my child to participate in all regular camp activities unless I specify restrictions in the health history section.

6. Behavior and Discipline Acknowledgment
I understand that my child is expected to follow camp rules, respect staff instructions, and behave in a manner that supports a safe and positive environment for all campers. I acknowledge that reasonable disciplinary measures may be used if my child’s behavior becomes unsafe, disruptive, or harmful.

7. Communication and Safety
I understand that camp staff may contact me or my emergency contacts if my child becomes ill, injured, or requires early pickup. I understand that if camp staff determine that my child must be picked up for health, behavior, or safety reasons, I (or an authorized adult) agree to arrange for reasonably prompt pickup upon request, taking into account the travel time to the campsite.
